Join Professor Nigel Pitts – Chief Strategy and Advocacy Officer at the ACFF, and Dr Maphefo Thekiso – Dental Specialist in Community Dentistry at the University of Witwatersrand as they discuss the continuing major challenge of Early Childhood Caries (ECC) and share details of the 2025 South Africa Policy Lab.
The ACFF is a global not-for-profit organisation that seeks to promote integrated clinical and public health action to confront the burden of tooth decay, fight dental caries initiation and progression, and, along with a global community of supporters, progress toward a Cavity-Free Future for all age groups. The South Africa Policy Lab held in Cape Town in January 2025 was the latest Policy Lab in a series hosted internationally by the ACFF.
For over 20 years, significant ECC has affected 60% of 6-year-old children in South Africa. Professor Pitts and Dr Thekiso, as part of the South Africa Policy Lab Planning Committee, brought together diverse stakeholders from across South Africa to tailor innovative pilots that focus on measurable impact in caries prevention and care for those aged 0-6, with each pilot designed with a view to sustainable national rollout.
The Policy Lab was completed over an intense 24-hour period, with the spirit of putting the mouth back in the body, working with existing health and education systems, and ensuring that no child is left behind.
ACFF extends its thanks to all attendees of the South Africa Policy Lab, who approached the Lab with enthusiasm and selfless teamwork. Some of their thoughts are below:

This IAPD session will present the process and results of the South Africa Policy Lab, evolving plans for the implementation of the Lab’s recommendations, and also share new details of how best to measure ECC from the age of 2 – 6 years and the policy consensus about how this burden can be prevented and managed.

🌐Spotlight on IAPD Consensus Recommendations
This month, we’re shining a spotlight on the Consensus Recommendation on Paediatric Dental Imaging Recommendations.
Visual inspection remains the essential first step in paediatric dental examinations and should precede radiographic imaging. Radiographs are indicated when further diagnostic accuracy is required; for example, in detecting caries, eruption problems, assessing treatment response, periodontal or dentoalveolar pathology, trauma, or developmental concerns. While radiation-free diagnostic methods exist for caries detection, radiographs remain indispensable for other indications.
Timing, selection, and frequency of radiographs should be tailored to the child’s individual clinical needs and align with national or local guidelines, always prioritising both diagnostic accuracy and radiation safety.

Through her Baby Clinic project, a pre- and postnatal initiative, she has made significant strides in promoting oral health for families and young children.
