Executive Summary

Minimal Invasive Dentistry (MID) focuses on the prevention, early detection, and arrest of dental caries, prioritising biological management and preservation of tooth structure over extensive surgical intervention.

Core principles include:

  • Caries arrest and risk-based management
  • Early detection and caries risk assessment
  • Remineralisation of demineralised enamel and dentine
  • Use of minimally invasive surgical techniques when necessary
  • Repair rather than replacement of restorations
  • Integration of preventive strategies alongside restorative care

The goal of MID is to preserve tooth vitality, minimise tissue removal, and promote long-term oral health outcomes in children.

Evidence-Based Recommendations

  • Preventive measures for caries arrest should include topical fluoride exposure and pit and fissure sealants where indicated.

  • The use of silver diamine fluoride (SDF) is effective in arresting cavitated carious lesions.

Consensus-Based Recommendations

  • Minimally invasive approaches such as resin infiltration are effective in the management of non-cavitated approximal dentinal lesions.

When surgical intervention is necessary, cavity designs should be conservative, emphasising minimal removal of carious dentine approximating the pulp and the use of adhesive restorative materials to restore function.
